You're survey is well put together. One concern for you would be that you don't have enough questions to formulate an analysis but I may be wrong. Also, it could be more helpful to have some open ended questions so you can really get the viewpoints from the survey taker. Other than that I have no complaints, just make sure you being drafting soon because breaking down each crime could be lengthy.
